Provides an array of services to support adults, such as adult case management, service coordination and referral, and supportive counseling.
The services are delivered in a variety of home, office, community-based depending on the client's needs, including Behavioral Health Home model.

Must have a Mental Health Rehabilitation Technician/Community certification or be eligible for a provisional (MHRT/C) and be able to obtain a full MHRT/C within three years from date of hire
Must have a Behavioral Health Professional (BHP) certification or be able to obtain a full BHP within one (1) year from date of hire
This position requires a minimum of one (1) year of relevant work or internship experience with adults, children and families.
